*Convalescent hospitals are facilities licensed to provide convalescent care and treatment for sick or injured patients on an in-patient basis. If you are pregnant at the time of application, we will pay for up to 2 days of hospitalization due to pregnancy.
$300 per year for each type of practitioner and combined maximum up to $650 per calendar year.
